Log Volunteering hours

I hereby authorize the Youth in Action to use my photograph(s), in part or in full, in any or all of its official publicity pieces without limitation. Publicity pieces include, but are not limited to, articles, news releases, promotional publications, website use and Facebook page. I relinquish the right to inspect or approve the final version prior to publication. I agree that there are to be no fees, commissions or royalties paid to me for the use of the photograph(s).
